Copy birth certificates

There are several ways you can order a copy birth certificate:

• On-line - request a copy certificate
• By telephone on 08450530021
• By mail : postal applications to:

  Preston PR2 2ZP
• Or in person at the above address ( View map and directions ). Please be aware when applying in person as there may be a waiting time whilst the certificate is being produced.

Cheques or postal orders should be made payable to Lancashire County Council.

Types of birth certificates that are available

Standard Birth Certificate
This certificate will give a complete copy of the birth entry to include name at birth, date of birth, place of birth and any parent details.
Required for: passports and other legal purposes.
Cost: £7.00

Short Birth Certificate
This certificate will show only the name and surname, sex, date of birth and district of birth.
Cost: £5.50

Adoption Certificates
Records of Adoptions in England and Wales are held by (External) The Registrar General .

Information that you will need

You must have the following information to obtain a copy birth certificate:

• Full Name at Birth
• Date of birth
• Place of Birth - (full address or name of Hospital)
• Fathers full name
• Mothers full name at time of childs birth - (including maiden name)
