The task of Lancashire County Council's Adult and Community Services is to contribute its skills, knowledge and resources to building sustainable local communities. We do this by
Our aim is to ensure that the majority of people’s needs can be met through ordinary, everyday community based services that are accessible to all. For those people with intensive or complex support needs, the task of community health and social care services is to enable them to access mainstream services wherever possible. Short term support, designed to help people gain or regain sufficient skills to manage their own lives, is a key task.
We will work in partnership with Primary Care Trusts, District and Parish Councils, voluntary, community and faith sectors, independent sector agencies, neighbourhood and community groups to develop local networks of universal services for the benefit of the whole adult population - and the promotion of sustainable communities. Together with work on person-centred enablement, they will result in better outcomes for individuals.
Helping people to undertake their own self assessment and purchase their own self-directed services, through personalised services, individualised budgets and direct payments will be the norm for most individuals. This will result in people having more choice, more control and a bigger say over the support they need. Our roles will change to ensure that people’s expectations are able to be met and that engagement with communities takes place. Technology will be used wherever possible to help people maintain their independence in the community.
Access to information about services available will be made much easier and effective signposting, in conjunction with other community services, will enable people to get the advice they need from range of community-based resources. Integrated health and social care teams will be located in a wide variety of community-based resources.
